Close your browser.
Go to FDM menu Options | Settings | Downloads | Monitoring and add FDM to the browser context menu. If the option is already checked, turn it off and on again.
You should also click there the "Customize..." button and check what options for the context menu are selected.

The bug is that sometimes FDM will lose my download completely from it's history and lose all progress as if I never even started the download. Now I'm pretty sure the problem is in background tasks (defragmenting, antivirus scanning etc.) that are scheduled to run by night. It seems that you download huge files (ISO images, compressed installers etc.) and don't change AV settings to not scan them.

Close Firefox, go to FDM menu Options | Settings | Monitoring and turn on Firefox monitoring. If it is already checked, turn it off an on again. There is no guarantee FDM can catch all possible dowloads, so please report download links that FDM could not catch.
